Application Form

When completing the application form, ensure all sections are completed. Application forms not fully completed will not be considered.

Application Criteria:

• Applicants must be a Nurse who is or has been registered with the Nursing Council of New Zealand or a Midwife who is or has been registered with the Midwifery Council of New Zealand.

• Applicants must have worked in the area covered by the Waitemata, Auckland or Counties Manukau District Health Boards (or their predecessors or successors) and currently reside in that area.
